What's Already Been Done: A Century of Quiet Life Extension

It's easy to focus on the frontier — gene editing, reprogramming, senolytics — and forget that life extension has already happened, dramatically, within the last 150 years. Global average life expectancy roughly doubled over that period, and the story of how is mostly unglamorous: sanitation, vaccination, antibiotics, clean water, and the decline of childhood infectious disease.

That matters for two reasons. First, it's a reminder that "life extension" isn't a speculative future category — it's an ongoing, measurable trend that's already delivered enormous gains, largely through public health infrastructure rather than personalized medicine. Second, it shapes what the next gains look like: the easy, population-wide wins from sanitation and vaccination have largely been captured in wealthy countries, which is part of why current research has shifted toward the biology of aging itself as the next frontier.

Within medicine specifically, several concrete advances already extend healthy years for large populations: statins and blood pressure management have measurably cut cardiovascular mortality; cancer screening and improved oncology have shifted survival curves for many cancers; joint replacement and modern surgery keep people mobile and independent for longer; and continuous glucose monitoring and better diabetes management have reduced complications that used to shorten lives significantly.

On the research side specifically targeting aging biology, a few results stand out as genuinely established rather than speculative: caloric restriction reliably extends lifespan in many model organisms (yeast, worms, flies, mice), though results in primates have been more mixed and human trials are difficult to run for obvious reasons. Rapamycin extends lifespan in mice in well-replicated studies. And several senolytic compounds have shown they can clear senescent cells and improve markers of healthspan in animal models.

None of this is immortality, and none of it is even formally approved as an "anti-aging" treatment. But it establishes the baseline this whole field builds from: humans already live roughly twice as long as they did in 1900, mostly through unglamorous public health work, and the current research wave is a bet that biology-targeted interventions can add a further, more evenly distributed set of gains on top of that.
